What To Do When High

What to do when high and anxious

You are safe. Cannabis anxiety is uncomfortable but temporary. No one has died from a cannabis overdose. The intense feeling will pass within 30 to 90 minutes for smoking or vaping, and within a few hours for edibles. Here is the 5-minute protocol that works.

Open the breathing tool

Do this right now, in order:

  1. Sit or lie down. Somewhere familiar. Lights low. No mirrors.
  2. Breathe 4-7-8. Inhale through nose for 4 seconds. Hold for 7. Exhale through mouth for 8. Four times.
  3. Sip cool water. Slowly. Just a few sips.
  4. 5-4-3-2-1 grounding. Look around and name 5 things you can see, 4 you can touch, 3 you hear, 2 you smell, 1 you taste.
  5. Put on a comfort movie or familiar album. Headphones if you have them.

If your heart is still racing after 5 minutes, repeat the breathing. The peak passes. You are okay.

Why anxiety happens when high

High doses of THC can over-activate the amygdala, the brain's threat-detection center. This is why your heart races, your thoughts loop, and ordinary sensations feel intense. CBD, which often accompanies THC in flower but not in concentrates or edibles, normally tempers this effect. High-THC, low-CBD products are the most common cause of acute cannabis anxiety.

It is mechanical, not psychological. Your body is doing something normal in response to a high dose. It is not a sign that something is wrong with you.

What helps within minutes

  • 4-7-8 breathing. Lowers heart rate via parasympathetic activation.
  • 5-4-3-2-1 grounding. Pulls attention out of the spiral.
  • CBD oil under the tongue. Often reduces THC intensity within 15-30 minutes.
  • Black pepper. Chew or sniff. Beta-caryophyllene may blunt anxiety.
  • Cold water on the wrists or neck. Resets the nervous system.
  • A familiar voice. Call a friend. Hearing someone calm helps.

What to avoid

  • More cannabis
  • Alcohol on top (worsens dizziness and anxiety)
  • Caffeine (raises heart rate further)
  • Scary movies, news, true crime
  • Reading WebMD or symptom-checking
  • Big decisions, important texts, social media
  • Hot, crowded, or chaotic environments

How long does cannabis anxiety last?

The intense feeling passes within 30-90 minutes for smoking or vaping. Edibles can take 4-8 hours. Sleep is the fastest reset.

Can cannabis cause panic attacks?

Yes, high-THC products in sensitive people can trigger acute panic. Cannabis itself is not lethal and panic attacks always end. Use 4-7-8 breathing and 5-4-3-2-1 grounding. If panic attacks recur, consider lower-THC or CBD-balanced products, or talk to a doctor.

Should I avoid cannabis if I'm prone to anxiety?

Many anxious people find low-dose CBD-balanced cannabis helpful, while high-THC products worsen anxiety. Start low (2.5-5 mg THC), avoid concentrates and high-dose edibles, and stop if it consistently makes you feel worse.

When should I get medical help?

Call SAMHSA 1-800-662-4357 (US) or Health Link 811 (Canada) if you have chest pain that doesn't pass after 20 minutes, repeated vomiting, fainting, or hallucinations that frighten you.
